What does vitamin B12 do?

The second enzyme processes a byproduct of fat and protein metabolism, and when B12 runs low, that byproduct backs up into a compound called methylmalonic acid, or MMA.

This biochemistry is not trivia; it is the reason the two best confirmation tests exist. When B12 is inadequate at the cellular level, homocysteine rises because the first reaction stalls, and MMA rises because the second one does. MMA is the more specific of the two, since homocysteine can climb for other reasons.1

What happens when you run low?

A shortage of B12 shows up in two systems, blood and nerves, and they do not always move together. In the blood, it causes a form of anemia with oversized red cells. In the nervous system, it causes tingling and numbness in the hands and feet, balance and gait problems, and in advanced cases a specific spinal-cord condition, along with memory and mood changes.

Here is the part most people miss: the nerve damage can appear with a normal blood count, so waiting for anemia to show up is the wrong trigger. In a classic study of people with nerve and cognitive problems from B12 deficiency, about 1 in 7 had a normal blood count, with no anemia and no enlargement of the red cells.2 And while early nerve damage usually reverses with treatment, damage from a deficiency left too long can be permanent.1 That asymmetry, cheap to catch and costly to miss, is the whole case for paying attention.

Who is at risk?

B12 comes almost only from animal foods, so the first at-risk group is vegans and vegetarians who do not supplement, among whom deficiency is common in surveys.9 But diet is only one route, and the absorption problems matter just as much:

  • Long-term metformin. The most relevant driver for people managing blood sugar. In a randomized trial, people taking metformin for over four years saw their B12 fall by about a fifth compared with placebo, with roughly 1 extra case of deficiency for every 14 people treated, and a long-term analysis found the odds rising with each year of use.54
  • Acid-blocking medication. Stomach acid is needed to free B12 from food, so long-term proton pump inhibitors and H2 blockers are linked to a higher risk of deficiency.6
  • Older age and stomach conditions. Aging stomachs make less acid, and pernicious anemia, an autoimmune condition that blocks B12 absorption, is a classic cause that needs lifelong treatment.
  • Gut surgery and disease. Gastric bypass or sleeve surgery, and conditions affecting the last part of the small intestine like Crohn's, impair absorption.

Why a normal B12 can miss a deficiency

This is the heart of the matter. The standard test measures total B12 in the blood, and it is a blunt instrument. Its normal range is wide and varies by lab, and the number can read normal in someone whose cells are truly short, or read low in someone who is fine, such as during pregnancy or on the birth control pill. A leading guideline puts it plainly: there is no perfect single test for B12, and the clinical picture matters most.3

The trouble concentrates in a grey zone, roughly the lower third of the normal range. A clearly high B12 makes deficiency unlikely and a very low one makes it likely, but a large middle band tells you little on its own. That is where a confirmation test earns its place, most of all when symptoms or risk factors are present.

What MMA and the active-B12 test add

Two tests look past the total number to how much B12 your cells are receiving.

The first is methylmalonic acid, or MMA, the compound that builds up when the fat-and-protein enzyme lacks B12. Because it reflects the shortage at the cellular level, MMA is more specific than homocysteine, which can rise from low folate, low B6, kidney problems, or a sluggish thyroid. The caveat is that MMA also rises when the kidneys are impaired, so it is read alongside kidney function.

The second is active B12, also called holotranscobalamin, which measures only the fraction of B12 that can reach cells. In a large accuracy study, both active B12 and MMA edged out the standard total B12 for catching early deficiency, though the margins were modest.10 In practice, an MMA is the usual next step when a total B12 falls in the grey zone with symptoms or risk factors, and it settles the question.

Folate and B12 work together, and a high folate intake can fix the anemia of B12 deficiency while doing nothing for the nerve damage, which then keeps progressing out of sight. In older Americans, having low B12 together with high folate was linked to worse anemia and cognition than low B12 with normal folate.11 The practical lesson: avoid loading up on high-dose folic acid without knowing your B12 status, because you can remove the warning light while the problem grows.

Do B12 shots boost energy?

Not if you are not deficient. B12 is water-soluble and hard to overdose on, but low risk is not the same as benefit. In someone whose B12 is already adequate, extra B12 by pill or injection has not been shown to raise energy, mood, or performance; the B12 shot for fatigue is a marketing idea more than a clinical one. If you are truly deficient, replacing B12 can help a great deal; if you are not, it does nothing.

One more point in the other direction: a very high B12 that you did not supplement can be a warning sign rather than a trophy. In a study of over 300,000 people, an unexplained high B12 was linked to a higher near-term cancer risk, because a high level can be an early marker of an underlying illness worth investigating.8

How is B12 deficiency treated?

For most people, treatment is simpler than expected. High-dose oral B12, in the range of 1,000 to 2,000 micrograms a day, works for many, even some with absorption problems, because a small fraction is absorbed passively without the usual machinery, and trials found it comparable to injections for most patients.7 Injections are preferred when the deficiency is severe, when there are significant nerve symptoms, in confirmed pernicious anemia, or when someone will not keep up with pills. Pernicious anemia needs lifelong replacement. The main point is that catching it earns you an easy fix, while missing it risks the hard, sometimes permanent, kind of damage.
