What is ferritin, and why is it so easily misread?

Ferritin is the protein your body uses to store iron, so in a healthy person the blood level roughly tracks how much iron you have in reserve. That is why a low ferritin is such a reliable sign of iron deficiency. But ferritin has a second job that trips everyone up: it is also what doctors call an acute-phase reactant, meaning it climbs in response to inflammation, infection, liver stress, and metabolic strain, whether or not your iron is high.5

This double duty is the reason a high ferritin is so often misunderstood. Across all the high results labs see, fewer than 1 in 10 turn out to reflect true iron overload; the rest are driven by inflammation or metabolic conditions.5 So a single high ferritin is a prompt to look closer rather than a diagnosis on its own.

What counts as a high ferritin?

Reference ranges vary by lab and by sex, but the usual upper limit of normal sits around 200 for women and 300 for men, in the units labs report (nanograms per milliliter). Mild elevations, into the few hundreds, are common and usually benign. A ferritin above 1,000 is a different matter and raises the possibility of organ involvement, most of all in the liver, which is why that level tends to trigger a referral.7 The rare extreme readings above 10,000 point to a short list of serious conditions and are handled urgently. For most people who land here from a wellness panel, the number is in the mildly-high range, and the question is what is driving it.

What does transferrin saturation tell you?

The single most useful next step after a high ferritin is a test called transferrin saturation, often shortened to TSAT, which measures how much of your iron-transport capacity is filled with iron. It is the test that separates the two big explanations.

A high ferritin together with a high transferrin saturation, above about 45%, points toward true iron overload, and calls for a check of the hemochromatosis gene.3 A high ferritin with a normal or low saturation, which is the far more common pattern, points away from iron overload and toward inflammation or a metabolic cause. In other words, ferritin tells you something is up, and transferrin saturation tells you which story you are in.

Could a high ferritin mean hemochromatosis?

It can, and hemochromatosis does deserve attention, but the odds are lower than the internet suggests. Hereditary hemochromatosis is the classic iron-overload disease, caused most often by carrying two copies of a variant in a gene called HFE. It is the most common genetic disorder in people of Northern European descent, affecting roughly 1 in 200 to 300, so it is not rare.3

Here is the nuance that calms most fears: carrying the genes does not mean you have the disease. In a large study that followed people with two copies of the main variant, only about 28% of men and about 1% of women went on to develop iron-overload-related illness.1 So even the highest-risk genotype leaves most people, and nearly all women, unaffected. At the same time, a UK study of nearly half a million people found that many who do carry a double dose go undiagnosed and slowly accumulate liver disease, diabetes, and joint problems, which is the argument for catching it when the iron numbers fit.2

When hemochromatosis does cause trouble, it shows up as fatigue, aching knuckles, liver damage, diabetes, heart rhythm problems, or a bronze tint to the skin. The good news is that the treatment is simple and effective: regularly removing blood, the same idea as donating, drains the excess iron, and when it is started before the liver scars, people live a normal lifespan.

The more common story: inflammation and metabolism

For most prevention-minded adults with a mildly high ferritin and a normal transferrin saturation, the cause is not their iron at all. It is metabolic. Ferritin runs high in insulin resistance, excess weight, type 2 diabetes, and fatty liver disease, a cluster so consistent that doctors have a name for it: dysmetabolic hyperferritinemia.10 In these cases ferritin is acting as a flag for metabolic and inflammatory strain, and in fatty liver specifically, a higher ferritin marks more advanced liver involvement.9

This reframes the whole result. If your ferritin is mildly high and your saturation is normal, the number is often telling you the same thing your waistline, your triglycerides, and your liver enzymes are telling you, that your metabolism needs attention. The fix is not to remove blood; it is to treat the root cause, the same work that improves your heart and liver health. Alcohol is another common driver, and any liver inflammation, recent illness, or thyroid problem can push ferritin up as well.

How should a high ferritin be worked up?

A sensible workup follows a clear order rather than guessing:

  • Repeat it. A single high value after a recent cold, a hard workout, or a few heavy-drinking days can settle on its own, so confirming the number comes first.
  • Check transferrin saturation and iron studies, the step that sorts iron overload from everything else.
  • Look at the common drivers: a marker of inflammation like CRP, liver enzymes, blood sugar and metabolic markers, and a candid alcohol history.
  • Follow the fork. A high saturation leads to hemochromatosis gene testing and, if iron is truly high, a liver MRI that measures iron directly. A normal saturation leads toward the metabolic and inflammatory causes.
  • Escalate when it is warranted. A ferritin above 1,000, abnormal liver tests, or confirmed iron overload should go to a liver specialist.4

Should you donate blood to lower ferritin or live longer?

This is a popular biohacking idea, and the evidence does not support it for people with normal iron. The theory is that iron fuels oxidative stress, so lowering it should protect the heart and extend life. But when this was tested in a randomized trial of over 1,200 patients, reducing iron by regular blood removal did not improve survival.6 And genetic studies point in conflicting directions: higher iron looks protective for coronary artery disease in one Mendelian randomization analysis,8 while other genetic analyses have linked higher iron to a greater risk of blood clots.

The fair read is that removing blood is the right treatment for confirmed iron overload and a poor bet as a longevity move for everyone else. Donating blood is a fine thing to do for others; doing it to chase a lower ferritin or a longer life is not backed by the trials.

What about a low ferritin?

Briefly, the flip side: a low ferritin is the earliest and most specific sign of iron deficiency, dropping before anemia ever appears, which makes it one of the most useful numbers on a panel. The catch is that because ferritin rises with inflammation, it can read falsely normal when iron deficiency and inflammation happen together, so a normal ferritin does not fully rule out deficiency in someone who is inflamed.
