TL;DR30-second take

Fishtown Medicine has no traditional office because most care happens better through secure messaging, video visits, and doorstep home visits. The mobile-first model keeps overhead low, response times fast, and care personalized to your real environment across Philadelphia.

TL;DR: Traditional healthcare forces you to fit into its schedule, fight traffic, hunt for parking, and sit in a germy waiting room. At Fishtown Medicine, we have skipped the waiting room. We are a mobile-first practice, providing care through secure messaging, video visits, and doorstep home visits throughout Philadelphia.

How has the doctor's visit evolved?

The doctor's visit has evolved from being a fixed appointment in a fixed location to a flexible interaction that fits real life.

  1. Text First: Most issues, from UTI symptoms to medication questions, can be handled with a quick message in the Ultralight app.
  2. Video Deep Dives: For more complex discussions, we move to a high-quality video consult where we review labs and imaging together.
  3. Doorstep Home Visits: When a physical exam is clinically necessary (lung sounds, ear checks, abdominal evaluation), Dr. Ash comes to you. We bring the clinical expertise to your living room or office.

Why is mobile-first care safer and smarter?

Mobile-first care is safer and smarter because it removes friction without sacrificing clinical depth.

FeatureThe Old WayThe Fishtown Medicine Way
LogisticsDriving, parking, waiting. From your couch or desk.
Infection RiskA room with other sick patients.Zero exposure, private care.
Appointment TimeRushed 15-minute visits.Unhurried, deep-dive planning.
CommunicationPhone tag with a receptionist.Direct access to your physician.

What if I need a physical exam?

You might wonder, "Can you really do a full exam at home?" Yes. We carry portable, clinical-grade diagnostics for:

  • Lung and heart auscultation.
  • Ear, nose, and throat evaluations.
  • Point-of-care rapid testing (flu, strep, COVID, mono).
  • At-home blood pressure calibration and monitoring.
  • Point-of-care ultrasound for select cases.

Performing exams in your environment also lets us see the real you, not the stressed version of you in a sterile clinic.

Guidelines from the Clinic

Dr. Ash
"A physical office is a barrier, not a requirement. By removing the overhead of a traditional clinic, I can spend my time where it matters: on you. My patients love that they can reach me while finishing their coffee or during a lunch break. We've brought the old-school house call into the 21st century with precision technology."

Actionable Steps for Frictionless Care

Make the most of the no-office model.

  1. Download the Ultralight app: The app is the secure hub for messaging, video visits, and document sharing.
  2. Pin Your Location: Add your home and work addresses to your profile for fast doorstep visits.
  3. Text First: Got a question? Send a message. We triage from there.
  4. Set Up Wearables: Connect your Apple Watch, Oura, or Whoop so we can see context in addition to your message.
  5. Schedule Quarterly Check-Ins: Use video for routine reviews, save in-person visits for moments that need a hands-on exam.

Frequently Asked Questions

Common Questions

Fishtown Medicine does not have a fixed clinic, but we serve all of Philadelphia, including Fishtown, Northern Liberties, Old City, Rittenhouse, Fairmount, and the Main Line. Our care happens in your home, your office, or by video. Mailing and administrative inquiries route through our Philadelphia headquarters.
A doctor's office can work without a physical office because most primary care is conversational, diagnostic, and decision-based. We use secure messaging for triage, video visits for deep discussion, and home visits when hands-on assessment is needed. Modern portable diagnostics fill any clinical gap.
Yes, the model is HIPAA compliant. We use encrypted messaging, secure video platforms, and clinical-grade documentation. Privacy and clinical safety are central to the design.
You may occasionally need to go to a physical clinic for things outside our scope, such as MRI, CT scans, mammography, or surgical procedures. We coordinate those visits at preferred Philadelphia centers (Penn, Jefferson, Temple). For everyday care, you do not need to go anywhere.
Home visits work like a scheduled appointment. We pick a time, confirm the address, and Dr. Ash arrives with a discreet clinical kit. Visits typically run 60 to 90 minutes. Home visits within Philly are included in the Executive tier.
We provide care across the Greater Philadelphia metro, including the Main Line, Mt. Airy, Chestnut Hill, parts of South Jersey, and parts of Delaware. We also offer travel-based home visits for members across the country.
For urgent care, message us first. We triage in real time, often resolve issues over messaging or video, and arrange a same-day home visit if needed. For true emergencies, call 911 first and message us when you are stable.
We handle prescriptions electronically. We send scripts directly to your preferred pharmacy through our EHR. We can also send refills, controlled substances (per our policy), and specialty medications without you ever needing to come in.

Deep-Dive Questions

Dr. Ash chose a no-office model because traditional clinic overhead drives short visits and slow responses. Removing rent, staff, and waiting-room expenses frees up time and resources for the patient. The model also lets us see patients in their real environment, which improves clinical insight.
Most primary care diagnostics can happen outside a traditional office: physical exam tools (otoscope, stethoscope, blood pressure cuff, ECG, point-of-care ultrasound), point-of-care testing (flu, strep, COVID, A1c), phlebotomy for any standard or advanced lab, and wearable data integration. The few exceptions (MRI, CT, mammography) require a fixed imaging facility.
No, no office does not mean no continuity. Continuity comes from the relationship between physician and patient, not the location. Members report feeling more continuity here than at traditional offices because they always reach the same person who knows their story.
The model fits Medicine 3.0 by extending care beyond reactive office visits into ongoing, proactive partnership. Wearables, messaging, and home visits let us track and support patients between visits, not just during them. Prevention happens in real life, not in a clinic chair.
Yes, the model serves complex chronic disease well. Members with diabetes, autoimmune conditions, complex GI issues, and post-hospital recovery often benefit from frequent messaging, home blood draws, and tight specialist coordination that traditional offices cannot match.
We focus on adult internal medicine, so pediatric primary care goes to a pediatrician. We support member families with adult preventive care and can advise on coordination during pregnancy or postpartum. We do not provide primary obstetric care.
Home visits are HIPAA compliant. We follow the same privacy standards used in any clinic, with secure documentation, encrypted communication, and trained staff. The smaller team size strengthens privacy.
During a major weather event, video visits and messaging continue as usual. Home visits may be paused for safety. We notify members in advance when weather will affect schedule. Most issues handle just fine over a screen.
The model fits busy households particularly well. Parents avoid waiting rooms, sick visits happen by message or quick video, and home visits prevent the chaos of dragging a sick adult or child to the clinic. Many of our most active members are 30 to 45 with kids.
We maintain rapport through frequent, direct communication. Members hear from Dr. Ash through messages and video far more often than in a typical practice. Plus, in-person home visits provide deeper face-to-face time than a 15-minute office slot ever could.
Most routine procedures happen at home. We administer vaccines (annual flu, Tdap, shingles, travel boosters) on home visits. Skin biopsies, cryotherapy for warts, and minor cyst drainage can be done in your home with the same sterile technique used in a clinic. We refer to dermatology or surgery for anything needing specialized equipment.
The model saves money over time by reducing avoidable ER visits, urgent care copays, hospital admissions, and time away from work. The flat membership fee replaces the patchwork of copays. HSA or FSA dollars cover the membership pre-tax, which adds another effective discount.
