TL;DR · 30-second take

The Strategic Roadmap is a five-visit deep-dive package at Fishtown Medicine for adults who want a comprehensive workup without committing to ongoing membership. It costs {{PRICE_PACKAGE_ROADMAP}}, runs across roughly three to four months, and covers a 90-minute history visit, advanced labs read together, a written care plan, plus follow-through visits. Many patients use it as a one-time deep look or as the bridge into membership.

The Strategic Roadmap: A Real Deep-Dive Without the Commitment

Some people want a comprehensive primary care workup but are not ready (or not the right fit) for ongoing membership. The Strategic Roadmap is built for them.

What it is

A five-visit package that runs across roughly three to four months. The same depth of evaluation as a new member's first 90 days, packaged as a defined product instead of an open-ended relationship.
WhatWhen
Visit 1 — History deep-dive90 minutes. Full history, prior records read together, family risk, goals.
Visit 2 — Targeted labs orderedAdvanced markers chosen for your specific picture (ApoB, Lp(a), fasting insulin, full thyroid, hs-CRP, etc.).
Visit 3 — Lab review60 to 90 minutes. Numbers read with you. The plan written together.
Visit 4 — Follow-throughFirst 30-day check-in. Plan tuning. Specialist referrals if needed.
Visit 5 — Re-test + closeRe-test the markers worth re-testing. Final written plan you keep.
You walk away with a written care plan, your advanced labs, and a clear next-step recommendation. Many patients use this exact path to decide whether membership is right for them.

What it costs

$975, paid once. Labs are billed separately at insurance or cash, whichever is cheaper for you. (We help you decide which path is cheaper before anything significant is ordered.) Smaller packages also exist if a five-visit arc is more than you want:
  • The Reset — $750, three visits.
  • The Audit — $395, one visit.
Full pricing on the packages page.

Who this is for

The honest profile of a great Strategic Roadmap patient:
  • Adults 35 to 65. The window where prevention compounds most.
  • You want a real workup, not a 15-minute physical. ApoB, Lp(a), fasting insulin, wearable review, family history factored in.
  • You are not ready to commit to a membership. You are testing the practice. You want to know if our thinking and your goals match.
  • You have a primary care doctor you intend to keep. The Roadmap fits cleanly alongside your existing PCP relationship.
  • You want a clean written plan you can take to your existing doctor or revisit on your own.
If you want ongoing access, same-day text, acute care, and a continuous relationship, membership is the better path. The Roadmap is finite by design.

What it does NOT include

So you go in with eyes open:
  • No acute care. If you get sick mid-Roadmap, you call your existing PCP or urgent care. The Roadmap is preventive and diagnostic.
  • No ongoing access between visits. Messages between visits go through the practice line and are not the same as the direct text members get.
  • No prescription refills outside the plan. Your PCP keeps prescribing what they prescribe.
  • No specialist coordination after close. We write the plan; you and your PCP execute it after the five visits end.

How to start

A free 20-minute call. We figure out whether the Roadmap is the right entry point for you, or whether membership or a smaller package fits better. About a third of Roadmap patients end up converting to membership; about two-thirds stay with their existing PCP and use the plan from the package as the structure for the next year. Both are good outcomes.
